If you don’t have a first name or it is unavailable, please enter the abbreviation FNU in this field and provide the last name as it appears on your ID and passport.

If you don’t have a surname, please use the abbreviation FNU in your name and enter your name as it appears on your ID and passport in the surname field.

Please write the name, leaving a blank space where the hyphen would be placed.

Just write your surname two times.

Please append the suffix to your last name without leaving a space.

If your first and last name together exceed 46 characters, please provide your first name and use initials for the additional names. Please enter your complete surname if it fits in the designated field. If it does not fit, please write your first name followed by the initial(s) of your other surname(s).
